Concrete cancer begins when water finds its way through cracks or pores in the concrete to the steel reinforcing inside, and reacts with it to form rust. The University of New South Wales's own explanation is worth reading in full: the rust is a problem twice over, because the corrosion product is bigger than the metal from which it formed, and because the portion of the material that provides the strength is corroding.
